30 min Consultation Free Initial phone conversation discussing why you need healthcare advocacy services and determine if Lifeline Patient Advocates can assist. n/a
Initial Assessment $300 If it is determined that Lifeline Patient Advocates is able to help the client’s situation, then an initial assessment is scheduled. In-person meeting lasting 1-2 hours reviewing the client’s health history, home situation, needs, medication list, healthcare concerns, healthcare goals, develop a healthcare plan, and review next steps. Prior to the assessment meeting (upfront)
Retainer $500 If the client wants to continue having Lifeline Patient Advocates assist, liaise, and advocate for his/her healthcare, then a retainer fee may be purchased. Once payment for the retainer fee is received and the contract is signed and fully executed, services will begin. The client will receive an itemized invoice detailing how the retainer fee is being used and how much money is left on the current retainer. Once the retainer fee is depleted, the retainer must be replenished for the continuation of services. Prior to receiving continued services (upfront)
On-going Advocacy $125/hour Applies to all advocacy services in-person or remote, expect for insurance advocacy services. Clients may purchase additional blocks of hours in advance or opt to pay as they go on an hourly basis for services. Payments by cash, check, and credit card are accepted. Prior to receiving continued services (upfront)
Travel Time $50 Clients will be charged for the advocate’s travel time. Will be deducted from the retainer

Lifeline Patient Advocates Services & Fees

All pricing indicated above is subject to change.

Professional hourly rates will be deducted from the retainer on a weekly basis. Once the retainer is depleted, the retainer must be replenished for the continuation of services. Patient Advocacy Services are not reimbursed by insurance companies or government agencies.

Reduced Rate

Due to the nature of your situation, you may be offered a reduced rate for services with the exception of the Assessment Fee. The reduced rate will only affect the retainer fee(s), as there is no fee for the initial consultation and we do not offer a discount for the initial assessment. As we discussed and agreed, the retainer fee(s) is being calculated at ____% or ____ (fraction) of $125/hour (reduced rate) and can be renewed at any time. Retainer fees are due prior to services being rendered.
